Seamless Hospital-to-Home Transition for Continuity of Care

When a patient requires home health services following a hospital stay, our team ensures a smooth and well-coordinated transition. We work closely with hospitals, inpatient care management teams, and clinical staff from various medical facilities in the region. These transitions can occur between a patient's home and the following settings:

  • Hospitals
  • Acute care facilities
  • Rehabilitation centers
  • Skilled nursing facilities
  • Assisted living facilities

We also offer home health services to residents of assisted living facilities. In most cases, patients can expect an initial home health visit from a nurse or therapist within 48 hours of their hospital discharge.

Personalized Care Plans

Once a patient is identified as needing home health care, a clinician will visit the home or assisted living facility to conduct an initial assessment. This assessment forms the basis for a personalized care plan.

The initial assessment covers:

  • Health status
  • Living conditions
  • Family/caregiver support
  • Current medications

This assessment is vital because we create individualized care plans based on the patient's specific healthcare requirements and the needs of their family or caregivers. Additionally, we discuss insurance requirements and financial expectations during this visit. It's important that the responsible party is present for this initial assessment.

Tailored Physician Communication

We recognize that every physician and referral source has unique preferences, and we are committed to delivering patient feedback in a manner that suits you best. You can specify the frequency and communication method for updates, whether it's through fax, phone, email, or other means.

How to Refer a Patient

Referring patients is easy for MWHC EpicCare Link providers. Learn more about MWHC EpicCare Link, a secure, online tool connecting physicians and clinical teams to MWHC resources and patient information in one electronic health record.

To make a referral, call 540.741.1667 and speak with Intake. We have a nurse available in the evenings and on weekends, accessible through our answering service.

Medicare and Medicaid have introduced updated requirements that physicians must follow before their patients can access home health services:

  • The referring physician (nurse practitioner, clinical nurse specialist, physician assistant) must have an "in-person" or face-to-face visit with all Medicare and Medicaid patients needing home health services.
  • The visit must have happened sometime in the 90 days before home health services are initiated or within 30 days after the home health nurse or therapist makes the first visit to the patient’s home.
  • The reason for the in-person visit must match the reason or problem the patient is receiving home health services.
