Mary Washington Neurology is a comprehensive and multi-specialty neurology practice, based in Fredericksburg, Virginia. Our highly trained neurologists provide the communities in central Virginia and greater Washington metropolitan area with academic level neurological care. We practice evidence-based medicine in the treatment and advances of neurological diseases and engage in clinical research. More importantly, we believe in an individualized, compassionate and personable approach to our patient care.

In addition to our outpatient practice, we are part of the physician group that evaluates and treats admitted patients at Mary Washington Hospital. We cover the hospital 24/7. We are all board certified neurologists who are either fellowship trained or have advanced specialty training. We work closely with other disciplines and experts that include neurosurgery, neuroradiology, physiatry (rehabilitation), neuropsychology and social workers. Although we are primarily adult neurologists, we are experienced in the evaluation and treatment of patients who are 18 years and older. We welcome new patients to our outpatient clinic.

Our Providers

Arnold Aguilera, MD

Dr. Arnold Aguilera is board certified in Neurology and in Clinical Neurophysiology. He is a member of the American Academy of Neurology and the American Association of Neuromuscular and Electrodiagnostic Medicine and is also a member of the Virginia Medical Society. He has contributed multiple articles and abstracts to the medical literature. His major interests are muscle diseases, peripheral polyneuropathies (diseases of the nerves in the limbs), entrapment neuropathies and electromyography (EMG/NCV). Prior to practicing in Virginia, Dr. Aguilera was director of MDA Neuromuscular Clinics at the Gundersen Clinic, LaCrosse, WI, and at the Geisinger Clinics in Danville, PA, and Wilkes-Barre, PA. As part of his neuromuscular practice, he performs muscle and nerve biopsies and skin biopsies for epidermal nerve fiber density. He has been recognized as a Top Doctor by Northern Virginia Magazine.

Dr. Aguilera also has a strong interest in headache and Parkinson’s disease and in the application of Botox therapy to neurological conditions including migraine headaches, hemifacial spasm, blepharospasm, cervical dystonia, spasticity and others.

Sandra Crouse, MD

Medical Director, Epilepsy Monitoring Unit

Dr. Sandra Crouse received her undergraduate degree from the University of Virginia and her medical degree from Eastern Virginia Medical School. Dr. Crouse completed an Internship in Internal Medicine at the Washington Hospital Center. She completed her Residency at Georgetown University Hospital and her Fellowship in Neurophysiology at the Georgetown University Hospital.

She has participated in research including nerve regeneration in diabetic peripheral neuropathy and is currently participating in a phase III/IV study in patients with partial onset epilepsy. She has interests in epilepsy, neuromuscular disorders (peripheral neuropathy, Myasthenia Gravis, ALS) and multiple sclerosis. Dr. Crouse is board certified in Neurology, Electrodiagnostic Medicine and Neurophysiology. She is a member of the American Academy of Neurology, American Academy of Electrodiagnostic Medicine and the American Epilepsy Society. Dr. Crouse is currently the at Mary Washington Healthcare.

Richard Erwin, MD

Dr. Richard Erwin is a board-certified neurologist. Dr. Erwin enjoys caring for patients with a wide variety of neurological conditions including multiple sclerosis, epilepsy, Parkinson’s disease, neuropathies, headache disorders and stroke. Having a special interest in stroke, he was instrumental in bringing cutting edge therapies to area stroke victims, serving as the Neurology Advisor on the Mary Washington Hospital Stroke Team from its inception in 1997, until stepping down in 2007. Dr. Erwin considers himself a true “general” neurologist; one who likes evaluating and treating all potential neurological concerns.

Dale Pcsolyar, MD

Dr. Dale W. Pcsolyar is board-certified in Neurology by the American Board of Psychiatry and Neurology (ABPN). Most recently he was certified as a multiple sclerosis specialist (MSCS) by the Consortium of Multiple Sclerosis Centers. He is a fellow and diplomate of the American Association of Neuromuscular and Electrodiagnostic Medicine (AANEM). In addition, he is a diplomate of the American Board of Pain Medicine and in neurosonology by the American Society of Neuroimaging.

He has served as Medical Director of the Neurodiagnostic lab at Mary Washington Hospital and as Director of the ICAVl-certified Cerebrovascular Lab of Fredericksburg. He is an active member of the American Academy of Neurology, American Heart Association- Stroke Council and the American Headache Society. In addition, he is a long-standing member of the AMA, Virginia Medical Society and Virginia Neurological Society. He is recognized as a Top Doctor by Northern Virginia Magazine.

Dr. Pcsolyar has 15 years’ experience programming Deep Brain Stimulators (DBS) for Parkinson’s disease and Essential Tremor, as well as Vagal nerve stimulators (VNS) for intractable Epilepsy. In addition, he administers therapeutic Botox for movement disorders, spasticity and chronic migraine. He has 30 years of experience performing EMG/NCV studies, as well as managing multiple sclerosis, migraine headaches and performing assessments for stroke risk prevention.

Nirali Soni, MD

Dr. Nirali Soni is a board-certified neurologist with interest in treating a wide range of neurologic ailments. She has specialized training in epilepsy and has an interest in treating new onset and complex epilepsy cases. Dr. Soni has also treated hundreds of headache patients and has expertise in Botox treatment for migraines and other emerging therapies.

With extensive education and experience in neurology, Dr. Soni provides sound clinical assessment and recommendation of appropriate treatment for patients suffering from various neurological conditions. Dr. Soni has participated in multiple research ventures including studies related to sleep apnea in patients with epilepsy, effects of adjunctive lacosamide on mood states, and quality of life for epileptic patients. Her priorities are patient care and patient satisfaction.

Carl J. Waldman, MD

Dr. Carl J. Waldman is a board-certified neurologist with special interests in adult neurology and clinical neurophysiology. He is a graduate of Northwestern University Feinberg School of Medicine. He completed a Neurology residency at Georgetown University Medical School and a fellowship in Clinical neurophysiology at Medical College of VCU.Dr. Waldman is a member of American Academy of Neurology, American Association of Neuromuscular and Electrodiagnostic Medicine, Medical Society of Virginia, and Former President (2013) of Arlington County Medical Society. He was awarded Top Doctors honors in Northern Virginia Magazine in 2011, 2012, 2015, and 2016. Most recently, Dr. Waldman was an attending neurologist at Penn Medicine, Clinical Care Associates of West Chester, PA.

Rachel Coggins, DO

Physical Medicine

Within the practice, Dr. Rachel Coggins focuses on non-interventional rehabilitation medicine for patients in need of stroke and cancer rehabilitation therapies. She also provides treatment for patients in the outpatient setting with muscle and joint injuries, and other disabling conditions. Dr. Coggins provides both inpatient consults and outpatient musculoskeletal clinics.

She completed an internship at St. Mary Mercy Livonia in Livonia, MI, and a physical medicine and rehabilitation residency at the University of Virginia in Charlottesville, VA. Dr. Coggins earned her medical degree from the University of Pikeville-Kentucky College of Osteopathic Medicine.

Dr. Coggins is a member of the American Academy of Physical Medicine and Rehabilitation, and the American Osteopathic College of Physical Medicine and Rehabilitation. Outside of work she enjoys reading, cooking, crafting, and hiking with her dog.

Dr. Rachel Coggins leads the interdisciplinary team at Mary Washington Healthcare which includes physical, occupational, and speech therapists, nurses, psychologists, and social workers. Working together, we draw upon the latest medical research and scientific evidence to develop a personalized plan designed to achieve your treatment goals. All care is provided in the comfort of our office. Here, you’ll find a caring, compassionate, outpatient environment that promotes wellness and healing.

  • If your insurance is an HMO or requires pre-authorization please obtain insurance authorization through your primary care physician. Our FAX number is 540.899.1359.
  • Co-pay, if applicable, is due prior to each visit. We accept cash, check, Visa, MC, American Express and Discover.
  • All completed paperwork and forms (Copies of forms may be found on our Patient Forms and Information page)
  • If you are unable to keep your appointment or need to reschedule, please call the office.
  • There is a $40 charge for missed appointments.

Referral from a doctor’s office: The physician’s office needs to fax the Physician Referral form or download our Neurology Consultation Form from our website and fax either the form to our office at 540.899.1359. Once the form has been faxed, our office will contact the patient and schedule the appointment. In addition, the patient can contact our office directly at 540.899.1354 to confirm the appointment date.
