Dr. Cook attended West Virginia University where he earned his undergraduate degree in 1989 and his medical degree in 1993. Dr. Cook completed his internship and family practice residency at Grant Medical Center in Columbus, Ohio. Dr. Cook is a board-certified family practitioner.

Dr Cook is always a kind listener and meticulous checking me over. His nurse Ko Ko is wonderful , calm, listening and very pleasant and polite . Everyone at the front desk was also pleasant and polite .
