Vaccine Card Replacement

1. If you have MyChart and received your COVID-19 immunization at Mary Washington Healthcare, a record of your immunizations is available by choosing the Health Summary menu item then selecting the Immunizations tab. You can access the scannable QR code for your vaccinations under My Record, then COVID-19. In addition, your vaccine information can be pinned to your MyChart lock screen.
2. If you received your vaccine in Virginia, you can email the Virginia Immunization Information System help desk at vaccinerecord@vdh.virginia.gov, or call the VIIS help desk at 877-VAX-IN-VA to request a copy of the card from 8:30 a.m.–5:30 p.m., Monday through Friday.
3. If you received your vaccine in Virginia, you can download your vaccine card online. Note: there must be a phone number associated with your record in order to complete your request.
4. You can also download a scannable QR code of your vaccine record if your employer needs to verify your vaccination status using the free SMART Health Verifier App. QR codes contain the same information as paper records, but in a format that offers greater security and efficiency. Because the QR code is digitally signed by the Virginia Department of Health, it cannot be altered or forged.
