Senior Care Services offers geriatric evaluations for older adults who are experiencing significant problems, such as memory loss, changes in personality, excessive falling, depression or other signs of change.
We take our time During a patient’s first visit, we’ll conduct an in-depth, two-hour evaluation to determine needs, answer questions and provide direction for obtaining specialized care. We request that the patient’s caregiver attend this evaluation.
Services offered
- Comprehensive assessment that focuses on cognitive, psychosocial and physical issues that may interfere with a senior’s ability to live independently
- Assessments conducted by a nationally certified nurse practitioner
- Focus on preventive measures and preservation of cognitive and physical function
- Coordination of care with primary care physician
- Evaluations for mobility, memory loss, driving and socialization issues
- Discussions of living wills and other ethical issues that may occur
- Inpatient consultation service available for patients at Mary Washington Hospital and Stafford Hospital
- Information on community resources
