For many patients, care does not end at hospital discharge or after leaving the doctor's office. Mary Washington Healthcare offers services to meet a wide variety of needs. Our home health agency provides skilled nursing assistance to patients who need periodic check-ups. Mary Washington Hospital Home Health has been providing professional specialty care to clients in our community since 1982. We are fully accredited by The Joint Commission for the Accreditation of Healthcare Organizations (TJC), plus we are Medicare-certified. Services include care from a variety of professionally trained specialists.
What we offer Home health services are offered to assist patients with the transition from an inpatient setting to home. Care is managed by each patient’s physician, in coordination with our staff. Services offered include comprehensive nursing care, rehabilitation services including physical, speech, and occupational therapy, dietary counseling, social work and caregiver support.
How our services work
- The patient’s primary physician writes a medical order for home health services. Services are intermittent, as needed.
- A plan of care, unique to each patient, is coordinated with the patient, family/caregiver and home health staff.
- Services are provided in the client's place of residence by home health staff.
