We strongly recommend you bring a list of all medications you are taking at home, whether prescription, over-the-counter medications, or nutritional or herbal supplements, whenever you visit our hospitals. To help you keep track of all your medications, consider using the universal medication form to list all your medications. Always keep an updated copy of this form with you and make sure you take it with you to all your medical appointments. Copies of this form can also be obtained from your physician.
